Invasion Of The Body Invaders is the final level of Eve's Asylum. The level is designed as a top-down shooter taking place within Dr. Herbert Higginbotham's body, where players must destroy the infectious meanies inhabiting Dr. Higginbotham's cavities through the use of white blood cell projectiles.

Plot overview
Eve Silva Paragorica and Sackboy tend to Dr. Herbert Higginbotham, whose thoughts are muddled and incomprehensible from the severity of his infection. Eve explains the urgency of their mission, before providing Sackboy with a shrinking device and robot capable of harnessing Dr. Higginbotham's white blood cells. Administered inside Dr. Higginbotham's mouth, Eve joins Sackboy briefly to demonstrate the cells' destructive power against the meanie infection blocking the passages of Dr. Higginbotham's head. Sackboy travels up through Dr. Higginbotham's sinuses and into his brain, before fighting off the cancerous meanie corrupting his mind.

Invasion Of The Body Invaders is a top-down shooter, where you control a robot with the ability to fly around Dr. Herbert Higginbotham's head through moving the
left stick. In order to progress through the level, you must use the white blood cells scattered around; white blood cells can be grabbed by pressing the
R1 button when near and releasing the button to launch the blood cell in the direction you are facing. Launched blood cells destroy pink meanie blockages and enemies upon contact at high velocities. Touching meanies depletes your robot's health; touching meanies four times without replenishing your health causes you to die and respawn at the nearest checkpoint.
The level begins in Dr. Higginbotham's mouth, where four prize bubbles are placed around the open space. Heading upward leads the player to Eve, who demonstrates how to destroy meanies by using the white blood cells. Clearing the meanies blocking the path up to Dr. Higginbotham's sinuses leads to four prize bubbles and multiple score bubbles. Once inside the sinuses, two blockages constantly shoot out meanie enemies that pursue the player; the left blockage protects a stash of prize bubbles, while the right blockage stops the player from progressing upward. Shooting white blood cells at the blockages multiple times clears them out and opens the path to Dr. Higginbotham's brain.
Dr. Higginbotham's brain holds the level's final boss — a large, dividing meanie. The meanie covers itself in a large coating that enlarges its damaging area, before moving around the circular arena in random patterns. Launching white blood cells at the meanie destroys this coating and damages it, after which it will gather a new coating around itself. Occasionally, the pink beam telegraphing the meanie's direction of movement will turn red, and it will snarl as it begins to directly pursue the player. After dealing enough damage to the meanie, it will divide in two, with its smaller halves gaining their own protective coatings, before moving in similar patterns as before. After one of its halves has been destroyed completely, and the other has taken enough damage, the meanie will split into four smaller parts, all of which gain their own coatings. Destroying all but one of the four parts causes it to chase the player endlessly until being destroyed.
